YOUR COMMITMENT AS A MEMBER OF TEMPLE BETH SHALOM
By submitting this form, I/we agree to the following:

- Tickets issued will be used only by members and their children under the age of 26 and are not transferable. These High Holy Day tickets obligate me to the 2024/2025 full year's membership dues as approved by our Board of Trustees.
- Tickets will be issued only to members in good standing who are current in their financial obligations for the fiscal year that ended on June 30, 2024. If you have extenuating circumstances or are unsure of the status of your financial account, please contact our financial office at 973-992-3600.

High Holy Day GUEST TICKETS for extended family may be purchased for $180 per ticket.
This includes married children, unmarried children age 26 and older (as of September 1, 2024), parents, grandparents, or any other relatives. Please also include children of your extended family ages 13-25, however there is no charge for their tickets.

PLEASE NOTE: We are unable to accept ticket requests for former TBS members and their families.

PAYMENT FOR GUEST TICKETS MUST BE RECEIVED IN THE TEMPLE OFFICE BY Tuesday, September 3.
